While scheduling your appointment, our staff will need to obtain necessary information; such as the nature of your appointment and your insurance information. In order for our schedulers to efficiently assist you, please have this information ready. Once your appointment has been scheduled, please direct yourself to the “Patient Forms” page here on our website. Our medical team is eager to get you on the path to wellness as quickly as possible. To streamline this process, please fill out the following forms prior to the arrival of your New Patient appointment: Registration, Authorization for Release of Health Information, Acknowledgement of Receipt of HIPAA Practices, Office & Financial Policies, Payment & Collection Policies, and the Adult or Pediatric Health History.
